The vascular interface

Between circulating blood and the vascular wall sits a functional interface only micrometres thick: the endothelial glycocalyx and the endothelial surface beneath it. Coagulation control, fibrinolysis, inflammatory signalling, permeability, cellular adhesion and shear-stress sensing all happen at — or through — this boundary.

These are not isolated systems. When the glycocalyx is damaged, coagulation interaction, permeability, inflammation and cellular adhesion can all change together, because they share the same physical stage. Treating the interface as one molecular system is the frame of the Panacea Sulodexide programme.

Sulodexide is scientifically interesting in this frame because its two glycosaminoglycan fractions interact with several of these systems at once: antithrombin III, heparin cofactor II, thrombin regulation, fibrinolytic balance, and — in human mechanistic and experimental research — glycocalyx-related measurements and permeability.

The scientific target is not indiscriminate anticoagulation. It is restoration of controlled interaction at the vascular interface.
